Hub Sites

We offer community Hub sites throughout Saratoga County where people come together to socialize, plan their day, share interests and experiences and develop skills. Each person with his/her family and staff develop an Annual Plan that establishes the person’s interests, skills, and dreams. The plan establishes goals to develop the competencies to achieve the desired outcomes. The annual plan includes a comprehensive schedule of activities and services. The goals are reviewed monthly by day habilitation staff and reviewed by the entire team at least semi-annually. Saratoga Bridges provides transportation to our programs.
