BENEFITS SUMMARY SHEET
HOLIDAYS...12 paid holidays: 8 scheduled and 4 floating. Holiday premium pay provided for employees required to work holiday.
VACATION...2 weeks after 1,2,and 3 years employment, 3 weeks after 4 years, and 4 weeks after 7 years of employment. 1 additional day for each year of service beginning with completion of 10 years of employment to a maximum of 5 additional days.
SICK LEAVE...earn one (1) sick day upon completion of first month of employment and one day per month for every month thereafter. Sick leave may be accrued up to the hourly equivalent of 140 days.
SICK LEAVE BUY BACK...A fulltime employee may elect to receive cash payment for unused sick leave up to the hourly equivalent of five sick days during a calendar year. To be eligible for the sick leave buy back a minimum number of hours must be available in the employee’s sick leave accruals following the buy back. It is based on your workweek as follows: 30 hours – 360; 35 hours – 420; 37.5 hours – 450; 40 hours – 480. The buy back is available on employee’s anniversary date.
SICK LEAVE BANK...Employees may donate up to 1 week of sick leave to the sick leave bank. Eligible staff with serious personal illness can request up to eight weeks of sick leave from the bank.
PERSONAL DAY...at completion of 3 months employment, earn 3 personal days. Leave is not accumulated from year to year, day credited on anniversary date.
BEREAVEMENT LEAVE...up to 3 days leave with pay will be granted to employees for death in the immediate family. Additional 2 days available in certain circumstances.
HEALTH INSURANCE...regular full time employees are covered at no cost under the MVP 25/40 Plan. CDPHP 25/40 is also available but a contribution is required. Dependent coverage is available under both plans but a contribution is necessary. Part time employees who work 20 to 29 hours per week are eligible but a contribution is required based on a pro-rated formula. Coverage begins the 1st of the month following 3 months employment.
DENTAL INSURANCE...regular employees, who work 20 hours or more per week, are eligible to enroll in a dental insurance plan. Individual or family coverage is available. The cost is the responsibility of the employee through a payroll deduction. Coverage begins the 1st of the month following 3 months employment.
DEFENSIVE DRIVING COURSE…National Traffic Safety Institute course is offered to all staff which provides a reduction in auto insurance premiums and point reduction on motor vehicle drivers’ license.$500
CASH STIPEND...regular full time employees that have other coverage and elect not to participate in the Agency's health insurance are eligible for a $42.00 cash stipend each month subject to income taxes.
FLEXIBLE BENEFIT PLAN...after 3 months of employment, regular employees working 20 hours or more per week may participate in a salary reduction and income tax free reimbursement plan for child care, health expenses and health insurance premium conversion expense.
HEALTH REIMBURSEMENT ARRANGEMENT…January 1st, 2007, one thousand sixty-nine dollars ($1069.00) is available to reimburse eligible staff for out of pocket expenses not covered by health or dental insurance plans. In subsequent years $750 will be available for the HRA.
DISABILITY INSURANCE…New York State statutory temporary income protection for an off-the-job injury or illness.
TERM LIFE INSURANCE...provided the 1st of the month after 3 months of employment to regular full time employees in the amount of $10,000.
RETIREMENT PLAN...after 2 years of continuous employment, employees 21 years or older working 20 hours or more per week are eligible for the Agency's 403(b) retirement plan. Annual contributions are made by the Agency on behalf of eligible employees. Employees can enroll and contribute to the Retirement Plan immediately upon joining the Agency. The plan is available through Benefit Plans Administrators (BPA) and the American Funds. Information on the Agency Retirement Plan is available in the Human Resource Office.
WORKERS' COMPENSATION...statutory income protection paid by the Agency for on-the-job accident. Provides income for the duration of the compensable injury or sickness.
EMPLOYEE ASSISTANCE PROGRAM...EAP provides confidential professional counseling and information services to help employees and eligible family members.
EDUCATIONAL ASSISTANCE...regular full time employees with 6 months of employment may submit written requests for tuition reimbursement of up to 50% of SUNY tuition rate.
CREDIT UNION...open to all employees. Provides savings and checking accounts, loans, mortgages, Christmas Clubs and credit cards. Employees can participate through payroll deductions.
DIRECT DEPOSIT…full service direct deposit is available to all staff. All or part of your net paycheck can be deposited into a maximum of 3 different accounts at 3 different financial institutions.
EMPLOYEE REFERRAL INCENTIVE…Staff members that refer an individual for employment shall be eligible to receive a cash award of five hundred dollars ($500.00). Regular full time, part time, substitute or per-diem staff members that refer an individual to become a Family Care Provider or an Individual Support Aide are eligible to receive a cash award of two hundred fifty dollars ($250) subject to withholding taxes.
EMPLOYEE MEAL BENEFIT...All residential staff members are eligible for one free meal per shift during weekday shifts, and up to three meals for each weekend day worked (meals are eaten family-style with consumers).
VOLUNTARY DEDUCTIONS
CANCER CARE INSURANCE...employees working 20 hours or more per week are eligible for cancer care insurance through AFLAC New York. Individual or family coverage is available. The cost is the responsibility of the employee through payroll deduction. Enrollment is completed in December of each year with an effective date of Jan. 1.
AFLAC ACCIDENT INDEMNITY PLAN…employees working 20 hours or more per week are eligible for accident insurance through AFLAC. Individual or family coverage is available. The cost is the responsibility of the employee through payroll deduction. Enrollment is completed in December of each year with an effective date of January 1.
SHORT/LONG TERM DISABILITY INSURANCE...in December of each year full time employees can purchase short/long term disability through payroll deduction. Provides income protection for off-the-job injury or illness. The cost is the sole responsibility of the employee.
WHOLE LIFE INSURANCE…regular full time employees are eligible to purchase a whole life insurance policy through payroll deductions. Enrollment is held in December of each year. The cost is the sole responsibility of the employee.
