Residence Counselor and Residential Habilitation Aide $10 - $11+/hr.

30-40 hours/Week, 52 Weeks/year, 2nd and 3rd shifts available. Must Have: High School diploma or GED. Valid NYS Driver’s license acceptable to Agency insurance standards (depending on program needs). Satisfactory completion of agency training program within six months of employment. Provide direct care to consumers receiving Agency Residential Program services. Through their actions, the employee must demonstrate an understanding of the person-centered philosophy of the agency; as well as its Mission and the Five Governing Principles. Staff will encourage and motivate consumers to foster personal choice, independence, inclusion, individuality and productivity in all areas of their lives. Ensures and provides for the safety and welfare of consumers at all times while allowing the dignity of risk when consumers make informed choices. Participates in the day to day operation of the residence by assisting in meal planning and preparation, inventory, shopping and purchasing of food and household supplies, and the cleaning of the residence, while actively involving the consumer to the greatest degree possible. Provides and/or assists with direct personal care to consumers including, but not limited to; feeding, dressing, toileting, and personal hygiene, while respecting the privacy and dignity of each consumer. Documents daily and/or unusual occurrences. Fosters community inclusion by involving the consumer in the planning and implementation of recreational and leisure time activities. Assists consumers and/or co-workers in performing lifting or transferring consumers to and from a wheelchair, bathroom facilities, therapy tables or bed, and performs any other lifts or transfers as required. Schedules, coordinates, and transports and/or escorts consumers to program, medical, dental, or other appointments as needed (unless designated as non-driving staff). Attends and participates in all agency required training sessions as per agency policy. Administers medications and treatments upon satisfactory completion of the medication administration course. Implements and carries out prescribed therapies or treatments as ordered by licensed physicians, nursing staff, or therapists. Performs behavior management (SCIP) when necessary. Demonstrates an understanding of person-centered philosophy by respecting and encouraging consumers to be as independent as possible, providing opportunities to make life choices, encouraging consumers to communicate their feelings and fears without the fear of harm or reprisal and ensuring that consumers receive supports and services which are effective and meet their needs .

Day Habilitation Assistant - $9.00+/hr, DOE.

35 Hours/Week; 52 Weeks/Year. High school diploma or equivalent. Experience within the human services field preferred. Must obtain certification to administer medications within six months of employment. A valid NYS driver's license acceptable to agency insurance standards. The Day Habilitation Assistant provides direct services as identified in the ISP to individuals enrolled in the HCBW day services. Supervised by the Day Habilitation Instructor I or Program Coordinator with regards to program policies, practices and procedures. Directly responsible to the Assistant Director or Director of Day Services. Implements the individual's service plan as scheduled by the Service Coordinator or Program Coordinator. Provides documentation of delivery of identified services. Provides supervision, training and assistance to consumers in developing appropriate social behaviors which take place in typical community settings such as appropriate conduct in restaurants, on public transportation vehicles, in recreational facilities, and in stores and other public places. Provides supervision, training and assistance to consumers in developing patterns of living, activities and routines which are appropriate to the individual's age and the practices of the surrounding community and which are consistent with the individual's interests and capabilities. Administers medication upon satisfactory completion of the medication administration course. Provides supervision, training and assistance to consumers in developing competency in personal hygiene such as bathing, toileting, dressing and grooming. P rovides supervision, training and assistance to consumers in developing health care skills. Provides supervision, training and assistance to consumers in developing basic skills in safety and in using public transportation. Visits appropriate volunteer or potential worksites to observe and develop possible volunteer or employments opportunities for participants. Provides one to one training to help the program participant acquire the necessary social and work skills to meet the volunteer coordinator or employer’s expectation (including role modeling/demonstration, shadowing/emotional support, systematic fading) and monitoring/evaluation work performance. Independently transfers service recipients to and from wheelchair, bathroom facilities, therapy table and performs any other transfers as required. Attends and participates in mandatory OMRDD approved training and Agency sponsored in service training as scheduled. Assists the consumer into and out of motor vehicles. Transports program participants, in personal auto or agency van. Performs all other duties deemed necessary by the Assistant Director of Day Services, Director of Day Services or Day Habilitation Services Coordinator.

Direct Care Relief reports directly to the Assistant Residence Manager and Residence Manager and is responsible to them with respect to questions regarding program policies, practices and procedures. This includes at least 120 hours of training during the first year of employment and all re-certifications and updates as directed thereafter. Additional Saratoga Bridges training and/or program specific training may be required to work at certain sits or shifts. May administer medications and treatments upon satisfactory completion of the medication administration course. However, medication administration is not required for substitute staff. Implements and carries out prescribed therapies or treatments as ordered by licensed physicians, nursing staff or therapists. Performs behavior management techniques (SCIP) when necessary after being trained and certified. Demonstrates an understanding of person-centered philosophy by respecting and encouraging consumers to be as independent as possible, providing opportunities to make life choices, encouraging consumers to communicate their feelings and fears without the fear of harm or reprisal, and ensuring that consumers receive supports and services which are effective and meet their needs. Bus Mechanic – Ballston Spa - $14.50/hr

40 Hours/Week, 52 Weeks/Year. High School Diploma or equivalent. Two years exp. in diesel/air brake maintenance preferred. A valid New York State driver’s license acceptable to agency insurance standards. Must obtain a CDL Class B with A and P endorsements, NYS inspector license, and Heavy Inspector License within twelve months of employment. Performs mechanical repairs to Saratoga Bridges motor vehicle fleet. Reports directly to the Head Mechanic. Performs monthly inspections of all vehicles, as required by NYS DOT regulations. Performs routine maintenance of Agency motor vehicle fleet. Completes repairs on vehicles as needed. Reports need for necessary repairs to Head Mechanic. Repairs and maintains lawn care equipment, snow blowers and fork lift. Performs the duties of a driver as needed. Independently lifts wheels, parts and equipment necessary for the repair of vehicles. Performs road service for breakdowns, accidents or other problems which may arise while vehicles are en-route. Participates in Agency In-Service Training programs. Completes all paperwork for vehicles worked on in accordance with NYS DOT regulations, NYS Registered Repair Shop regulations and Fleet inspection station. Removes license plates from junked vehicles and turns plates in to dispatcher. Inspects new vehicles. Maintains service area in safe and clean condition at all times. Performs all other duties as assigned by the Head Mechanic or Transportation Director.

Day Habilitation Instructor II – Clifton Park - $12/hr

35 Hours/Week; 52 Weeks/Year. Bachelor of Science degree in Human Services, Education, Special Education, Psychology or a related field. One year experience in day services preferred. A valid NYS Driver’s License acceptable to agency insurance standards. Responsible for the planning and delivery of day habilitation services for individuals enrolled in HCBS day waiver services. Reports to the Assistant Director of Day Services with regards to program policies, practices and procedures. Attends initial meetings with the consumer and his/her advocate and the consumer's Service Coordinator to discuss the services requested and begin developing a Day Services Habilitation Plan. Develops, writes and ensures proper implementation and documentation of Day Habilitation goals as identified by the individual service plan. Provides daily direction to Day Habilitation staff to ensure services are being provided as specified in the habilitation plan and that the consumer is satisfied with these services. Insures that consumer's chosen goals are implemented to improve or enhance the individual's independence, inclusion or integration into the community and that the outcomes are individualized. Attends meetings for individuals receiving day services and reviews or revises the Day Habilitation Plan as needed. Organizes Day Habilitation Services Program environment and maintains and requisitions program materials and supplies. Coordinates the delivery of day habilitation activities in the program and community. Administers medication upon satisfactory completion of the medication administration course at the request of Assistant Director of Day Services. Serves as a liaison with the appropriate landlord or the BOCES program to ensure that building and program space is monitored for safety and the needs of the program participants. Completes quality assurance activities in a timely manner. Provides direct assistance in all areas of individual care including, but not limited to, personal hygiene, dining and socialization skills. Plans for and adheres to the general health/safety practices necessary for the welfare of the consumer. Attends and participates in in service training. Independently transfers service recipients to and from wheelchair, bathroom facilities, therapy table and performs any other transfers as required. Assists the consumer into and out of motor vehicles. Transports program participants, in personal auto or agency van. Visits potential worksites to observe and perform jobs to learn responsibilities for a given position prior to the program participant’s placement. P rovides daily one-to-one training to aid the program participant in acquiring the work and interpersonal skills needed to meet the employers’ expectations for work performance and social functioning in the work setting (including role modeling/demonstration, shadowing/emotional support, systematic fading) and monitoring/evaluation work performance. Visits appropriate volunteer sites or potential worksites to observe and develop possible volunteer or employment opportunities for participants. Provides one to one training to help the program participants acquire the necessary social and work skills to meet the volunteer coordinator’s or employer’s expectations (including role modeling, emotional support, monitoring, evaluations). Performs any other duties deemed necessary by the Assistant Director of Day Services, Director of Day Services or Day Services Habilitation Coordinator.

Program Coordinator II – Clifton Park - $14.50+/hr, DOE

40 Hours/Week, 52 Weeks/Year. Bachelor of Science degree in Human Services, Education, Special Education, Psychology or a related field. Minimum of two (2) years work experience with the developmentally disabled including one (1) year in a supervisory capacity. Valid NYS drivers license acceptable to Agency Insurance. Responsible for the development, training and supervision of all programs for all assigned individuals. This includes the training, supervision and evaluation of staff in the area of implementation of programs and the ensurance of quality of programming through periodic review. Inclusive in this responsibility is coordination of all individual services directly related to their Individual Service Plan/CFA. Directly responsible to the Director of the Day Services Program with respect to questions regarding policies, practices and procedures. Supervised by the Assistant Director of Day Habilitation Services or Day Habilitation Services Coordinator. . Develops and/or reviews individual Service Plan (ISP) and/or Comprehensive Functional Assessment (ISP/CFA) and ensures it corresponds to the day services and residential plans, (ensures annual, physical, TB test, behavior plans, personnel protection training, self medication test, diets, medications, allergies, and safeguards are included). Oversees the development, training, supervision and evaluation of all classroom staff. Ensures all clinical services are developed and implemented by the IDT. Assists staff in the development of person centered outcomes which correspond with the individual ISP/CFA. Performs individual Quality Assurance checks in Day Services programs, ensuring that active treatment, active learning, and quality services are being provided. Attends and participates in individual ISP/CFA meetings. Assists with providing coverage for daily staffing patterns in the day services programs, routinely and or in the absence of direct care staff. Assists in locating potential worksites for individuals who want employment opportunities and placement. Oversees the writing of all non-clinical programs. Evaluates through informal observation and formal periodic evaluation of staff's performance implementing programs, developing person-centered strategies, and developing activities which correspond with the individual’s ISP/CFA. Acts as a liaison with outside providers. Oversees the development and accuracy of all program plans and ensures appropriate data collection for all program plans and clinical support. Ensures that monthly review of progress in all programs is completed in a timely manner. Ensures that programs are revised or changed as needed. Participates in in-service training on a regular basis as required. Performs in-services specific to goal planning, observation, recording and data collection. Reviews monthly calendar of activities and seeks out new contact and volunteer opportunities. Performs direct care duties as required or in the absence of direct care staff. Ensures the maintenance and accuracy of the universal record and all billing documents. Performs all other duties as directed by the Director of Day Services or the Assistant Director of Day Habilitation Services .
