Residence Counselor and Residential Habilitation Aide $10 - $11+/hr
30-40 hours/Week, 52 Weeks/year, 2nd and 3rd shifts available. Must Have: High School diploma or GED. Valid NYS Driver’s license acceptable to Agency insurance standards (depending on program needs). Satisfactory completion of agency training program within six months of employment. Provide direct care to consumers receiving Agency Residential Program services. Through their actions, the employee must demonstrate an understanding of the person-centered philosophy of the agency; as well as its Mission and the Five Governing Principles. Staff will encourage and motivate consumers to foster personal choice, independence, inclusion, individuality and productivity in all areas of their lives. Ensures and provides for the safety and welfare of consumers at all times while allowing the dignity of risk when consumers make informed choices. Participates in the day to day operation of the residence by assisting in meal planning and preparation, inventory, shopping and purchasing of food and household supplies, and the cleaning of the residence, while actively involving the consumer to the greatest degree possible. Provides and/or assists with direct personal care to consumers including, but not limited to; feeding, dressing, toileting, and personal hygiene, while respecting the privacy and dignity of each consumer. Documents daily and/or unusual occurrences. Fosters community inclusion by involving the consumer in the planning and implementation of recreational and leisure time activities. Assists consumers and/or co-workers in performing lifting or transferring consumers to and from a wheelchair, bathroom facilities, therapy tables or bed, and performs any other lifts or transfers as required. Schedules, coordinates, and transports and/or escorts consumers to program, medical, dental, or other appointments as needed (unless designated as non-driving staff). Attends and participates in all agency required training sessions as per agency policy. Administers medications and treatments upon satisfactory completion of the medication administration course. Implements and carries out prescribed therapies or treatments as ordered by licensed physicians, nursing staff, or therapists. Performs behavior management (SCIP) when necessary. Demonstrates an understanding of person-centered philosophy by respecting and encouraging consumers to be as independent as possible, providing opportunities to make life choices, encouraging consumers to communicate their feelings and fears without the fear of harm or reprisal and ensuring that consumers receive supports and services which are effective and meet their needs.

Day Habilitation Assistant - $9.00+/hr.
30-35 Hours/Week; 52 Weeks/Year. High school diploma or equivalent. Experience within the human services field preferred. Must obtain certification to administer medications within six months of employment. A valid NYS driver's license acceptable to agency insurance standards. The Day Habilitation Assistant provides direct services as identified in the ISP to individuals enrolled in the HCBW day services. Supervised by the Day Habilitation Instructor I or Program Coordinator with regards to program policies, practices and procedures. Directly responsible to the Assistant Director or Director of Day Services. Implements the individual's service plan as scheduled by the Service Coordinator or Program Coordinator. Provides documentation of delivery of identified services. Provides supervision, training and assistance to consumers in developing appropriate social behaviors which take place in typical community settings such as appropriate conduct in restaurants, on public transportation vehicles, in recreational facilities, and in stores and other public places. Provides supervision, training and assistance to consumers in developing patterns of living, activities and routines which are appropriate to the individual's age and the practices of the surrounding community and which are consistent with the individual's interests and capabilities. Administers medication upon satisfactory completion of the medication administration course. Provides supervision, training and assistance to consumers in developing competency in personal hygiene such as bathing, toileting, dressing and grooming. Provides supervision, training and assistance to consumers in developing health care skills. Provides supervision, training and assistance to consumers in developing basic skills in safety and in using public transportation. Visits appropriate volunteer or potential worksites to observe and develop possible volunteer or employments opportunities for participants. Provides one to one training to help the program participant acquire the necessary social and work skills to meet the volunteer coordinator or employer’s expectation (including role modeling/demonstration, shadowing/emotional support, systematic fading) and monitoring/evaluation work performance. Independently transfers service recipients to and from wheelchair, bathroom facilities, therapy table and performs any other transfers as required. Attends and participates in mandatory OPWDD approved training and Agency sponsored in service training as scheduled. Assists the consumer into and out of motor vehicles. Transports program participants, in personal auto or agency van. Performs all other duties deemed necessary by the Assistant Director of Day Services, Director of Day Services or Day Habilitation Services Coordinator.

Varied Hours. Work(s) a flexible schedule that varies in duration and frequency and is established by the In-Home Services Coordinator/Respite Coordinator and family in support of individual’s and family’s needs. High school diploma or G.E.D., a valid NYS Driver's License acceptable to agency insurance standards and satisfactory completion of agency/OPWDD training program within six months of employment. Personal vehicle may be required, depending on individual residential habilitation requirements. Provide companionship and assistance to individuals with developmental disabilities in the home/community as reflected by the individuals’ needs, choices and desires. The individual support staff functions in several areas of responsibility as noted in the following sections. The common theme is to provide varying levels of support to the individual with a developmental disability based on a clear understanding of the individual and their desires and choices. The individual can range in age from infant to adult. To provide in-home care for individuals with a developmental disability through companionship and other activities in recreation or leisure areas of interest. The provision of in-home care allows the parent(s) and other family members to attend to their respective task or interests knowing that their son, daughter or relative is safe and well cared for. Implements direct support at home and/or in the community for individuals with a developmental disability. These activities are detailed in the Individual Support Plan that supports the overall themes and outcomes within the individual’s life. The Residence Manager II works in a behaviorally involved home or supervises more than one group home and is directly responsible to the Assistant Director of Residential Services with respect to questions regarding program policies, practices or procedures.
