  Residence Counselor and Residential Habilitation Aide $9.00 - $9.90/hr.

25-40 hours/Week, 52 Weeks/year, 2nd and 3 rd shifts available. High School diploma or GED. Valid NYS Driver’s license acceptable to Agency insurance standards (depending on program needs). Satisfactory completion of agency training program within six months of employment. Provide direct care to consumers receiving Agency Residential Program services. The Residence Counselor reports directly to the Assistant Residence Manager and Residence Manager and is responsible to them with respect to questions regarding program policies, practices, and procedures. Through their actions, the employee must demonstrate an understanding of the person-centered philosophy of the agency; as well as its Mission and the Five Governing Principles. Staff will encourage and motivate consumers to foster personal choice, independence, inclusion, individuality and productivity in all areas of their lives. Ensures and provides for the safety and welfare of consumers at all times while allowing the dignity of risk when consumers make informed choices. Provides assistance and training for consumers in all personal skill areas through the implementation of the programmatic plans of service as agreed upon by the consumer, advocate and team. Participates in the day to day operation of the residence by assisting in meal planning and preparation, inventory, shopping and purchasing of food and household supplies, and the cleaning of the residence, while actively involving the consumer to the greatest degree possible. Documents daily and/or unusual occurrences including, but not limited to: fire drill performance, incident reports, program data sheets, time sheets, vehicle mileage sheets, daily log entries, bed checks, and any other documentation as requested by your supervisor. Participates as a member of a person-centered planning team by communicating the consumer’s interests, likes / dislikes, and by being a role model for the consumer. Implements and monitors programs by documenting progress towards the consumer’s valued outcomes. Fosters community inclusion by involving the consumer in the planning and implementation of recreational and leisure time activities. Assists consumers and/or co-workers in performing lifting or transferring consumers to and from a wheelchair, bathroom facilities, therapy tables or bed, and performs any other lifts or transfers as required. 25 – 35 Hours/Week Mon-Fri; 52 Weeks/Year. Locations: Clifton Park & Wilton. High school diploma or equivalent. Experience within the human services field preferred. The Day Habilitation Assistant provides direct services as identified in the ISP to individuals enrolled in the HCBW day services. Provides supervision, training and assistance to consumers in developing patterns of living, activities and routines which are appropriate to the individual's age and the practices of the surrounding community and which are consistent with the individual's interests and capabilities. Administers medication upon satisfactory completion of the medication administration course. Provides supervision, training and assistance to consumers in developing competency in personal hygiene such as bathing, toileting, dressing and grooming. Provides supervision, training and assistance to consumers in developing health care skills. Provides supervision, training and assistance to consumers in developing basic skills in safety and in using public transportation. Visits appropriate volunteer or potential worksites to observe and develop possible volunteer or employments opportunities for participants. Provides one to one training to help the program participant acquire the necessary social and work skills to meet the volunteer coordinator or employer’s expectation (including role modeling/demonstration, shadowing/emotional support, systematic fading) and monitoring/evaluation work performance. Independently transfers service recipients to and from wheelchair, bathroom facilities, therapy table and performs any other transfers as required. 35 Hours/Week; 52 Weeks/Year. Bachelor of Science degree in Human Services, Education, Special Education, Psychology or a related field. One year experience in day services preferred. Visits appropriate volunteer sites or potential worksites to observe and develop possible volunteer or employment opportunities for participants. Head Mechanic – Ballston Spa

40 Hours/Week, 52 Weeks/Year. High School Diploma or equivalent, 2 years experience in heavy duty, large mechanical repairs and a NYS CDL Class B with A and P endorsements meeting agency insurance standards. Must meet the Office of Mental Retardation Developmental Disabilities (OMRDD) regulations regarding Qualified Mental Retardation Professional (QMRP). Valid NYS driver’s license acceptable to Agency insurance standards. Directly responsible to the Director and Assistant Director of Residential Services with respect to agency and program policies, practices and procedures. Responsible for the development and monitoring of the Residential Habilitation Plan, Behavior Plan and Protective Oversight Plan for all assigned individuals. This includes the training, supervision and evaluation of residential staff in the implementation of support plans through periodic reviews. Inclusive in this responsibility is the coordination of all residential services directly related to the Individual’s Support Plan and having a working knowledge of State and Federal regulations.

1. Oversees the development, training and supervision of all residential staff by the following methods:

a. Participates in the development of: reviewing and updating the person’s Individual Support Plan in concert with the 3I’s and P and person centered philosophy. Coordinates the development of; reviewing, updating and signing the person’s Protective Oversight Plan, Res Hab Plan, DDPII or other relevant service plans. (Comprehensive Functional Assessments for ICF’s)

b Trains staff in the implementation and data collection of all individual support plans and services.

  • Evaluates through informal observation and periodic evaluation of staffs' performance implementing all residential support programs, including active treatment, choice and independence and the person’s progress in achieving their valued outcomes.

d. Evaluates the person’s satisfaction with activities, interventions and/or therapies. Ensures program integration through periodic observations of Individual’s Day Program.

2. Participates as a member of the interdisciplinary team through:

a. Participates in and attends individual oriented meetings, to include admission/discharge, 30-day reviews, annual and semi-annual meetings.

b. Reviews and recommends with the person and their team the revisions of outcomes, or selection of different outcomes/activities, interventions or therapies and specify the appropriate action to be taken and time-frame for achievement of the revised/changed outcome.

c. Identifies noteworthy observations relevant to the person’s receipt of services.

d. Ensures all routines and specialized services are delivered in a timely and appropriate fashion according to the individual’s ISP and/or CFA..

e. Coordinates services with outside agencies as appropriate.

3. Ensures the documentation of:

a. Appropriate data collection for all Residential Habilitation, Behavioral and/or other individual program plans.

b. A monthly review of the person’s progress is completed for all support plans and services

c. Completes 90-day, semi-annual and annual summaries or assessments of the person’s progress relative to his/her receipt of residential services in a timely manner.

40 Hours/Week, 52 Weeks/Year. Residential, On Call As Scheduled. Graduate of an accredited School of Nursing with current NYS license to practice as a Registered Professional Nurse. One year experience working as an RPN in an agency or facility for developmentally disabled individuals preferred. Valid NYS driver’s license acceptable to Agency insurance standards.

GENERAL RESPONSIBILITIES :

Performs a variety of nursing duties as part of interdisciplinary team member. Works under the direct supervision of Supervising Registered Nurse.

SPECIFIC RESPONSIBILITIES :

1. Functions as an interdisciplinary team member in the following way:

a. development of Pre-Admission evaluation

b. participation in 30-Day Post-Admission evaluation and formulation of a Health Care Plan

c. responsible for writing, implementing and monitoring nursing Health Care Plan

d. development of Discharge Plan

e. participation in Quarterly Reviews and physicals

f. participation in Annual Re-evaluations

g. the planning and implementation of the training of personnel

The Residence Manager II works in a behaviorally involved home or supervises more than one group home and is directly responsible to the Assistant Director of Residential Services with respect to questions regarding program policies, practices or procedures. The Residence Manager II is responsible for the overall day to day operation of the home. Documents and performs appropriate follow-up of all incidents within a 24 hour period. Ensures proper usage and recording of consumer and petty cash monies. Participates in residence financial audit. Ensures medications are ordered, received and stored in accordance with agency policy. Provides and/or assists with direct personal care to consumers including, but not limited to: feeding, dressing, toileting, and personal hygiene, while respecting the privacy and dignity of each consumer. Assists consumers and/or co-workers in performing lifting or transferring consumers to and from a wheelchair, bathroom facilities, therapy tables or bed, and performs any other lifts or transfers as required. Schedules, coordinates, and transports and/or escorts consumers to program, medical, dental, or other appointments as needed (unless designated as non-driving staff). Administers medications and treatments upon satisfactory completion of the medication administration course. Certification must be maintained through remedial training and annual recertification. Decertification will result in a reduction of pay. Implements and carries out prescribed therapies or treatments as ordered by licensed physicians, nursing staff, or therapists. Performs behavior management (SCIP) when necessary. Demonstrates an understanding of person-centered philosophy by respecting and encouraging consumers to be as independent as possible, providing opportunities to make life choices, encouraging consumers to communicate their feelings and fears without the fear of harm or reprisal, and ensuring that consumers receive supports and services which are effective and meet their needs. Performs all other duties as directed by the Director or Assistant Director of Residential Services.
