Family Support In-Home Respite

Funded by the Office of Mental Retardation and Developmental Disabilities, In-Home Respite is considered intermittent, temporary relief to the family who cares for a family member with a developmental disability. Most families receive an average of 4-6 hours per week, depending on their personal situation. The number of families that can be served is limited. We also provide in-home respite and for children enrolled in Early Intervention. Referrals come directly from the child’s service coordinator.
A trained Individual Support Aide will work directly in the family’s home following through with recommendations and goals defined by the family, which are stated on the individuals respite plan. Eligibility criteria specifies that the individual receiving the services has a diagnosed developmental disability and resides with their family in Saratoga County .
Recreation – Adult Social Club

The Social Club is also funded by a state grant through the Office of Mental Retardation and Developmental Disabilities. The Adult Social Club is open to all individuals who reside at home with their families, who reside independently in Saratoga County and who are 21 or over. The Social Club is supervised by a Coordinator and is staffed with Recreation Aides. The club plans their own activities and programs and often times plan larger trips on Saturdays. Transportation is provided, however, there are pick-up points throughout the county, and we encourage those who live locally to our destination to provide their own transportation. All activities are paid for by the participants, however, for larger events, we offer discounts. To request an application, you may contact Carin at the above numbers.
Saturday R & R (Respite & Recreation)

Saturday R & R is an exciting program for children with special needs, ages 6 through 12. This program is held at the Southern Saratoga YMCA. The program is offered in 7 week sessions in the fall, winter and spring from 9:00 AM – noon . Activities include: arts and crafts, creative movement, music, physical games and swimming. Children who reside in Saratoga County are welcome to apply. Eligibility includes a diagnosis of a developmental disability.
Community Outreach and Referral

This service assists families and individuals with accessing services and information regarding supports. Home visits can be scheduled with families to determine needs and referrals to providers will be offered. In addition, we offer support in Future Planning and Guardianship, Family Education and Training and Case Management as needed. A seasonal Family Support Services Newsletter is distributed among families, offering a wide array of information on activities, programs, contact information and articles of interest.
Home and Community Based Services Waiver Program, Residential Habilitation and Respite

Individuals enrolled in the HCBS Medicaid Waiver Program and have identified in their Individual Services Plan the need for in-home services, would be eligible for Residential Habilitation and Respite. Referrals for these services come directly from the Service Coordinator. A home visit is scheduled with the Service Coordinator and In-home Services Coordinator to determine interests and begin to identify goals. Individual Support Aides work directly with the coordinator and the individual and family to carry out the services identified in the Residential Habilitation or Respite Plan.
Family Reimbursement

The Family Reimbursement Program provides financial support to families to help assist them in the care of a family member with a developmental disability. Through an application and committee review process, the program can assist in the purchase of services and goods based on the individual and family needs.
Sibshops

Sibshops is a fun-filled support group for children ages 8 – 12 who have a sibling with special needs. Sibshops is offered twice per year – each session meets 4 times.
Family Care

Family Care is a Residential Program offering adults with developmental disabilities an opportunity to live with a family who is certified by New York State to provide 24-hour residential supports within their home. Family Care Providers must successfully complete a rigorous training and certification process, prior to having an individual live with them. Providers are located within Saratoga County .
