As the Program Coordinator II (Integrated Services) you will be responsible for: 

  • Participates in the Life Plan, and documents special team meeting minutes that include the individual and all team members
  • Orchestrates the development of person-centered, valued outcomes which correspond with the Life Plan ensuring all regulatory requirements are met
  • Oversees the development and accuracy of all program plans and ensures appropriate data collection for all services and supports through weekly data checks
  • Provides training and supervision to instructors and habilitation staff as it relates to implementation of all programmatic services and habilitation plans
  • Writes/ensures that monthly review of progress is completed
  • Works with person at Day Services and their home to ensure continuity of integrated services

Must haves:

  • Bachelor’s degree in Human Services, Education, Special Education, Psychology, or a related field
  • Two years of experience working with people who have a diagnosis of a developmental disability
  • Valid NYS driver’s license and use of personal vehicle to travel between worksites
