Community Habilitation

Community Habilitation is designed to encourage community integration utilizing existing local resources for purposes of skill building based on goals in an Individual Service Plan (ISP).  People enrolled in the Medicaid Waiver Program and deemed eligible for services by OPWDD may be considered for this service depending upon availability of service hours.  Referrals come directly from the Service Coordinator to the Assistant Director of Family Support Services.  A home visit may be scheduled to determine interests and begin to identify goals.  Individual Support Aides work directly with the individual and family to carry out the services identified in the Community Habilitation Plan.
