6pm-8pm

Saratoga Bridges

16 Saratoga Bridges Blvd., Ballston Spa

This training was developed to assist families in gaining a better understanding of MSC, in strengthening partnerships with their child’s Service Coordinator, and in becoming an effective advocate in securing necessary supports and services

Families will learn about:

  • The features and benefits of MSC
  • The rights and responsibilities of family members receiving MSC
  • The Individualized Service Plan – what it is and why it is important
  • How, together with your Service Coordinator, to advocate for and gain access to necessary services and supports

This will serve as one required FET for the year 2015.

If FET is your only waiver service, you may be in jeopardy of losing your Medicaid if you do not attend 1 training within the year!!

Please disseminate this FLYER to families who may be interested.
